The form below is shown to help reviewers and prospective customers see exactly what the SMS consent step looks like inside Fieldproxy. It does not submit anywhere and does not create an account. Real opt-in happens during account creation in the Fieldproxy web app and during first login in the Fieldproxy mobile app.

How this matches what users actually see
- The checkbox is unchecked by default. Account creation cannot complete unless the user explicitly checks it.
- The disclosure text shown next to the checkbox is identical to the text reproduced above and to the language published in our SMS Policy.
- We record the user's consent, IP address, and a timestamp at the moment the checkbox is checked.
- Users can opt out at any time by replying STOP to any SMS, or by removing their mobile number from account settings.
